Frequently Asked Questions

1. How do I get the necessary paperwork for my visits?

You have several options.   If the  corporate
office  is  not convenient  for  you,  we  can
fax  and  mail   you   paperwork.    We  are
currently in the process of  providing agen-
cies the option of  making  their paperwork
available to the  therapists  on our  website.
We also have convenient drop-off locations
where paperwork can be left for  therapists.

2. How do I submit my paperwork?

Paperwork is submitted weekly.   You may
drop  it  off at  our  corporate  office  or  at
a designated drop-off  location.   You  may
also mail  it  to our office.  Evaluation  fre-
quency must be called in and faxed by the
next day.

3. Can I choose where I want to work?

Yes.   We will map out the location  where
you want to work and assign  you referrals
based on that area.

4. How many patients can I expect?

The  number  of   patients  you  receive  is
based    on   your    availability,   and   the
geographic  location  you   want  to  work.
Naturally,  the more  flexible the  therapist
can be, the more referrals we can dispatch
to that therapists.

5. What is the contractual relationship between
    Timeless Therapy and the Therapists?

The therapists is an independent contractor providing   services  for  Timeless  Therapy.
Timeless  Therapy  compensates   the  ther-
apists at a contractural  rate per visit  and is
all inclusive. The therapists will complete a
W-9 Form and  Timeless Therapy will issue
a 1099 for services rendered.
